COMPREHENSIVE COMMUNITY SERVICE TEAMS

Comprehensive Community Service Teams (CCST) services take place in either an outpatient or community based setting and provide assistance in identifying goals and consumer choices to promote resiliency and facilitate recovery.  CCST services are intended to restore the individual’s function and participation in the community and to assist and guide individuals in reconnecting with the society, rebuilding skills in identified roles in their environment.  The focus is on individual strengths and resources as well as their readiness and phase of recovery.  A team approach of services will be used to guide and support individuals with development of a recovery plan focusing on the areas of individual and family living, learning, working and socialization.

Services provided include Assessment, Case Management, Supported Housing, Supported Employment and Day Services.  Description of Programs follow. 

Targeted Case Management

The mission of the Targeted Case Management is to ensure that all people in recovery receive the services they need to live successfully in the community and that these services are delivered in a timely and caring manner. Upon referral to the Case Management Program, each person is assigned a Case Manager who assesses his or her needs and develops a Comprehensive Service Plan (CSP) based on individual goals and service needs. The Case Manager links the individual to the appropriate service providers, monitors the effectiveness of the services delivered, and advocates on his or her behalf whenever assistance is required.

Boley Centers’ Case Management Department serves adults from Pinellas County who have a severe and persistent mental illness and meet eligibility requirements for Targeted Case Management services as defined in the 65E15 F.A.C.

Supported Housing Program

The Supported Housing/ Living Program assists individuals in finding appropriate housing alternatives of their choice in the community and provides the ongoing supports necessary to help them maintain their residency. Staff offer a wide range of services such as housing placement, tenant/landlord negotiations and mediation, independent living skills assistance, budgeting and financial assistance, crisis intervention and general in-home support. Services are primarily intended for individuals in need of intensive supports in order to obtain or maintain placement in the community.

Supported Housing is available to individuals and couples who are able to live independently in a non-supervised setting, with the help of support services.  This program offers permanent housing.  

The program’s housing is located primarily in St. Petersburg and south Pinellas County. Most of the apartments are one-bedroom units, while some of the homeless housing units are efficiencies and a few units offer two-bedroom, double occupancy.  

Most of the apartments are funded by the US Department of Housing and Urban Development (HUD). Proof of eligibility and a lease is required before moving in and a regular income is needed to cover living expenses.

Supported Housing staff are available to work with individuals to identify services needed to lead a more satisfying life.  This includes a variety of services such as medication management, budgeting, linkage with financial assistance, independent living skills, crisis intervention, roommate mediation, accessing community resources, emotional support, developing friendships and skills teaching. Individuals in supported housing are encouraged to be involved in meaningful activities such as school, day programs, work, vocational training or volunteer work.

Referral process:

Referrals can be made by contacting the Director of Case Management, or through the Department of Children and Families Alcohol, Drug Abuse and Mental Health Program Office. Any consumer not currently case managed who is entering a Level II or Level III licensed residential facility will be assigned a Boley Centers' case manager.
