Transitional Homeless Apartment Program
Description:
The Transitional Homeless Apartment Program provides a supervised apartment setting for homeless adults with mental illness. This program allows individuals who are homeless and mentally ill obtain psychiatric, vocational and other rehabilitative services needed to enhance their ability to become reintegrated into the community and maintain permanent housing.
Entry criteria:
To be eligible for entry into the Transitional Homeless Program, an individual must meet the following criteria:
- Be homeless according to the Department of HUD criteria.
- Must be 18 - 54 years of age
- Present psychological symptomatology consistent with ICD-9-CM and DSM IV (Axis I ) diagnosis of a major mental illness.
- Be able to participate in self-care and treatment, and to manage the effects of his/her illness.
- Not be currently dangerous to himself/herself or others

Hours of Operation: Facilities are in operation 24 hours a day.
